NAHUC Scholarship Application Form 

Criteria

1.  Health unit coordinator course or program completion within the last calendar year.

2.   Demonstrate commitment to health unit coordinating as a profession.

3.   Good academic standing. (3.0 on a 4.0 scale)

Procedure        

1.   Submit the completed personal data and following required documentation to the NAHUC office at the following address by September 30.

2.   Submit proof of health unit coordinator course or program completed.

3.   Submit a 50-100-word essay describing the professional goals of the applicant by September 30th by the NAHUC Office.  Incomplete applications will not be reviewed by the committee.
