Tape users agree to the terms below by completing the order form for tape use. All fees payable in U.S. Dollars
1. Complete the above form, and fax or mail with payment to NAHUC.
2. Tapes and post-tests will be mailed via USPS as they become available on a first-come first-serve basis. Post-tests may be photocopied.
3. Tapes are due back to the NAHUC office on date indicated on return form. Due date is assigned three weeks after request processed.
4. Return tapes and completed post-tests to NAHUC. Borrower is responsible for return postage.
5. Borrowers who do not return tapes or do not provide sufficient payment will be reported to the NAHUC Board of Directors for further action.
6. Credit cards will not be charged, other than for processing payment, unless tapes are not returned by deadline or tape is lost or damaged (borrower will be charged replacement cost if necessary) or for unpaid bills or returned checks.
7. Late fee: $10.00 per week, a one-week extension may be requested in writing prior to the due date.
